Personalized plan, wearable + lab integration, and expert guidance.

Step 0 — Sign Up & Onboarding (Day 0)

 

Welcome! Once you sign up, you’ll:

  • Complete a short intake form with your health history and goals.

  • Upload any recent labs or wearable data (optional but recommended).

  • Receive scheduling instructions for your 75-minute initial telehealth visit.

Goal: Get all the information we need to personalize your plan.

Step 1 — Pre-Visit Data Collection (Day 0–7)

 

Before your first visit, we’ll:

  • Review your intake form, labs, and wearable data.

  • Send lab ordering instructions if you choose optional baseline labs.

Your task: Complete labs and share wearable data before the visit to get the most accurate personalized plan.

Step 2 — Initial 75-Minute Telehealth Visit (Day 7–14)

 

During this deep-dive session, we will:

  • Review your labs, wearables, and lifestyle patterns (sleep, activity, stress).

  • Co-create your 90-day Longevity Plan, including nutrition targets, strength training, sleep routines, and recovery strategies.

  • Give you a one-page plan and shared dashboard for tracking your progress.

Goal: Set a clear, personalized roadmap for the next 90 days.

Step 3 — Week 1 Follow-Up & Tracking (Day 7–21)

 

After your first visit, we help you get started:

  • You’ll receive an implementation checklist for workouts, nutrition, and sleep.

  • We’ll show you how to log metrics from wearables and labs.

  • Quick check-ins to troubleshoot early challenges.

Goal: Make sure you’re confidently implementing your plan.

Step 4 — Midpoint Check-In (Day 30)

 

At 30 days, we review your progress:

  • Telehealth call (20–30 minutes): Assess sleep, energy, mood, and strength trends.

  • Celebrate wins and adjust areas that need fine-tuning.

  • Updated 30-day action plan with measurable goals for weeks 5–8.

Why it matters: Keeps your plan personalized and evolving with your progress.

Step 5 — Ongoing Support & Micro-Checks (Weeks 5–11)

 

We stay connected as you continue your plan:

  • Access to brief messaging for quick questions (limited to 2 messages/week).

  • Log your weekly metrics in your shared dashboard.

  • Receive guidance on adjustments and accountability reminders.

Goal: Keep you on track while minimizing unnecessary calls — support without overwhelm.

Step 6 — Final 90-Day Review & Longevity Report

 

At the end of the program, we provide:

  • 30–45 minute closing call reviewing your labs, wearables, and functional improvements.

  • Your 90-Day Longevity Report Card, showing key biomarker trends, functional gains, and next steps.

  • Option to transition into an ongoing membership program for continued optimization.

Goal: Celebrate your results and plan your next phase of longevity.

 

 

Step 7 — Optional Data Contribution

 

With your consent, we can include your de-identified data in our pilot dataset:

  • Helps improve the program and supports research.

  • Participants may receive early access or a small incentive for contributing.

Goal: Contribute to the longevity community while maintaining full privacy.

FAQ Section

  1. What labs do I need?      Recommended panels include metabolic, hormone, and inflammation markers.

  2. Do I need a wearable?    Optional but helps track sleep, HRV, and activity.

  3. Messaging support: Limited to 2 short messages per week; anything longer may convert to a paid visit.

  • All data is stored securely and HIPAA compliant.

  • De-identified data used only for internal pilot evaluation, with consent.

  • Pilot program designed by a licensed healthcare provider specializing in longevity and wellness optimization.

Payment, Refund, and Reschedule Policy

1. Payment

  • Full payment for the Longevity Optimization Assessment is required at the time of booking.

  • Payment secures your place in the queue and allows the clinician to begin reviewing your submitted data.

2. Refunds

  • All payments are non-refundable.

  • This is because your assessment is personalized and starts as soon as payment is received, including AI-assisted data analysis and clinician review.

3. Rescheduling

  • You may reschedule your telehealth consult up to 48 hours before your scheduled appointment without losing access to your assessment.

  • Rescheduling requests submitted less than 48 hours before your appointment may result in loss of the scheduled time, but your payment still applies to your assessment.

4. Missed Appointments

  • If you miss your scheduled consult without rescheduling in advance, your payment will not be refunded, and you will need to schedule a new consult using the same completed assessment.

5. Agreement

  • By completing payment, you acknowledge that you have read and agreed to this policy, and understand that payments are non-refundable.
